TEAM MOSAICS
Mobile Outreach, Stabilization, Assessment and Intensive Community Supports
TEAM MOSAICS is a program developed to provide intensive supports in the community through establishing a team of mental health professionals that provide a mosaic of services to support families that include, assessments, community psychiatric supports, psychoeducational services and the 24/7 Mobile SOS Team.
Assessment
An intake appointment, psychosocial history and various assessment tools and techniques will be utilized to help clients and their families identify needed supports and services within the MOSAICS Program, within Palomé or within the community. The assessment process is designed to partner with clients and identified supports in developing a treatment or service plan that best meets the behavioral health needs of the client. A family systems perspective is utilized to wrap services around the client and family and develop a support team to meet the goals established in the treatment or service plan.
High Fidelity Wraparound Services
High Fidelity Wraparound Services provide individualized, community-based, non-clinical interventions provided in conjunction with other services. Services are provided to a child, youth or young adult and their family members and natural supports to maintain or restore successful community living. A collaborative planning process is utilized to help stabilize and strengthen the placement of the child, youth or young adult in a community family setting.
Comprehensive Community Supports
Comprehensive Community Supports provide mental health rehabilitation and resiliency services within a team approach. Interventions are strengths based, promote stability in symptoms related to behavioral health challenges, increase coping skills and support relational skills with the intentions of maintaining the client within the community setting. Comprehensive Community Supports aim to provide support to decrease crisis episodes, increase independence in the community and increase community interaction. Some of the activities that are available through Comprehensive Community Supports are symptom assessment and management, supporting counseling, support with daily living skills, encouraging engagement in peer support services, development of transition goals and planning, advocacy, crisis intervention, education about mental health symptoms, facilitating access to health care, education in meeting transportation needs, safety and locating affordable housing.

TIPS for Teens
TIPS for Teens is an intensive outpatient program for adolescents ages 13-19 that offers multiple supports for both the adolescent and their families. Included within the TIPS for Teens Program are individual and family therapy services, a psychosocial rehabilitation program, advocacy services, family connections activities, high fidelity wraparound, parent and adolescent support groups and a service team.
Psychosocial Rehabilitation/Intensive Outpatient Program
TIPS for Teens offers a Psychosocial Rehabilitation/Intensive Outpatient Program for adolescents ages 12-19 that provides group-based services for four hours per day for up to four days per week depending on the adolescent’s needs. The Psychosocial Rehabilitation/Intensive Outpatient Program utilizes aspects of Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, Experiential Therapy, Art Therapy, and Music Therapy to teach anger and stress management skills, cognitive skills, social and interpersonal skills, and direct skills, independence, and autonomy. Youth are also able to explore aspects of identity, identify prosocial and healthy replacement behaviors and experience safe and positive relationships in a safe, trauma informed, nurturing setting.
Psychoeducational Services -TIPS for Parents
TIPS for Parents (Trauma Informed Parenting Supports) provides education and support to parents and caregivers of children and youth with difficult behaviors related to mental health or trauma. Education is provided on nurturing parenting skills, trust based relationship intervention and beyond consequences education in a supportive, non-judgmental setting to help build protective factors in families and work with the family as a system.
Individual, Family and Group Therapy
Individual, Family and Group Therapy is also available at Palomé to include adult and adolescent therapy, children’s therapy, and family therapy for all age groups.

24/7 Mobile SOS Team
A 24/7 resource is available for families while they are in the TEAM MOSAICS program that families can access in times of crisis. A team of trained mental health professionals are available 24/7 to provide mobile services and supports to families facing challenging behaviors and conflict within the home related to mental health and behavioral disorders. Our 24/7 Mobile SOS Team is available to reduce the demand on emergency services such as police officers or emergency room visits when the behavioral health crisis is able to be managed in the home. This service is not intended for use when there is a safety threat to self or others in the home. If client is suicidal or threatening to hurt others, emergency services should be accessed.
